Dear Referring Agency Representative,

We are now accepting profiles for the 2024 Holiday Season!  Here are some key details to consider when submitting profiles below.

New for 2024: To streamline our process, we ask that you work with other caseworkers within your organization to fill out the Agency Tracking From below with all pertinent family information (agency family number, children’s names and ages, parent’s name) before submitting profiles online. This collaborative approach will help ensure accuracy and maintain proper tracking throughout the process. Once completed, please submit the form via email to info@HeartToHeartST.org. 

 

 Here are a few important reminders and details for this year’s program:

Profile Submission:

• Please complete one child profile per child and one family profile per family unit.

• Use the “family number” you assigned on the attached agency tracking form when submitting profiles. It’s crucial that these numbers align to avoid any confusion.

• Complete the entire profile with as much detail as possible. Please keep in mind that our resources are limited, and all gifts are provided through the generosity of our donors.

• Be aware that we are unable to provide higher-priced items such as gaming systems or furniture.

   

Book Selection:

• A book summary list is attached with brief descriptions. Please review these with each family to help them select an appropriate book for each child who would like one.

      

Family Supply List:

• There is also a family supply list attached, including household items and family games. Each family should select one game and one household item, which should be included in the Family Profile.

Profile Submission Deadlines:
Profiles can be submitted online via Google Forms below (keep scrolling for Family profile). If you are unable to submit online, please contact us to discuss alternative arrangements.

 

We kindly request that all profiles be submitted by Friday September 20th, and sooner if possible.
